We are a network of cross-sector partners working across Charlotte, Collier, Glades, Hendry, and Lee counties to improve the health and well-being of every resident and visitor in Southwest Florida (SWFL). Our goal is to ensure that by 2040, SWFL exceeds the 90th percentile nationally for quality and length of life. We are committed to addressing social determinants of health, reducing health disparities, and empowering individuals to thrive.
